Post #58 is the version of this I will quote in future. One addition.

Source for the reading SURMOUNT-1 without the press figure, since it was asked for. It is in the discussion rather than the abstract, which is why the version circulating is stronger than the paper is.

Reading the surrounding paragraph is worth the two minutes. The authors are more careful than their summarisers.

SURMOUNT-4's randomised withdrawal design is the strongest available evidence about what happens on stopping. It says nothing about a lower maintenance dose, because the comparison was continue versus placebo rather than continue versus less.

That is the version I use. It may not be the version that is correct.

Building on post #63 rather than restating it.

What would change my mind on reading SURMOUNT-1 without the press is a second dataset collected by someone with no stake in the first. Until then I hold it loosely and I would rather say so than pretend to more.

The apnoea-hypopnoea index used in SURMOUNT-OSA is an objective measurement rather than a symptom scale, which is why that trial carries more weight than its size suggests. Two parallel trials with and without positive airway pressure addressed the obvious confounder directly.

Two claims get bundled together under reading SURMOUNT-1 without the press and they need separating. The descriptive one — this is what was observed — is usually well supported. The causal one — this is why — usually is not.

Almost every disagreement in threads like this one dissolves once you say which of the two you are making.

Weight reduction figures from SURMOUNT-1 are frequently quoted without the trial's duration attached. A mean change at 72 weeks and a mean change at 40 weeks are different numbers and both circulate.

I would keep reading SURMOUNT-1 without the press and the decision it usually gets used for separate in this thread. They are related and they are not the same question, and merging them is why the last one went badly.

Post #71 is right about the mechanism and I think understates the practical bit.

The practical version of reading SURMOUNT-1 without the press is three sentences long. The rigorous version is three pages and reaches the same conclusion with the conditions attached.

Reading SURMOUNT-1 without the press: I have looked for the primary source twice and failed twice. Either it does not exist or it is somewhere I do not know to look, and I would like to know which.

SURMOUNT-4 used a randomised withdrawal design: everyone titrated, then those on maintenance were randomised to continue or placebo. Continuation maintained effect; withdrawal was followed by regain. The finding is robust but it says nothing about a lower effective maintenance dose, because that was not studied.

The number people quote for reading SURMOUNT-1 without the press is a central estimate presented without its interval, and the interval is wide enough that the estimate is nearly uninformative on its own.

SURMOUNT-1 reported weight reduction of a magnitude that was the largest for a pharmacological intervention at that time of publication. It also reported a clear dose response across three doses. The categorical thresholds (people reaching 10%, 15%, 20% loss) got the most attention but read less informatively than the mean weight change.

Filing this under things that are true until someone shows me otherwise.

Filing a mild objection to the consensus on reading SURMOUNT-1 without the press. Mild because I might be wrong; an objection because nobody has addressed the case that does not fit.
