Journal club: the CagriSema phase 2 combination paper — a second dataset

CagriSema phase 2 combination paper is a good example of a question where the honest answer is boring and the interesting answers are unsupported. I would go with boring.

Journal club: the CagriSema phase 2 combination paper — a second dataset — setting out what I have, and where I think it stops being reliable.

Posting a small dataset on CagriSema phase 2 combination paper. It is mine, it is uncontrolled, and the method is stated so it can be discounted appropriately.

What I would like is not agreement but a second dataset collected by someone with no stake in mine. If one exists I would rather read it than argue for this one.

Helpful, and short, which on this subject is harder than long.

This follows post #2 rather than contradicting it.

The figures usually contain the finding and the text usually contains the interpretation. Separating them for the first twenty minutes is a discipline worth keeping.

Suggest the next paper at the end of the session rather than by message afterwards. The room has just worked out what it wants to know next.

Reading it again, the caveat matters more than the finding.

Sessions on negative or null results are consistently the most instructive and the hardest to get anyone to attend.

I have left out the parts I could not verify.

The thing about CagriSema phase 2 combination paper that took me longest to accept is that a plausible mechanism is not evidence of an effect. It is a reason to look, not a result.

Taking post #7 at face value and following it one step further.

The summary that goes to the digest should say what the paper establishes, what it does not, and one thing the group could not settle. Three paragraphs, no more.

Everything in post #7 holds. The case it does not cover is the one I have.

Somebody should be asked to argue the paper's case even if nobody in the room believes it. Sessions where everyone agrees produce a summary nobody can use.

Agreed on all of that, and I have nothing to add to it.

That is clearer than the version I had in my head. Thank you.

I read post #9 twice before replying, because I had assumed the opposite.

Session format: read the paper before posting. The discussion is much better when everyone has. Start with the estimand and population, then methods, then results, then limitations. That order makes critique coherent.

Adding it because I spent an afternoon working it out and nobody should have to twice.

Taking post #12 at face value and following it one step further.

The most common failure mode is spending fifty minutes on the effect size and ten on the population. Reversing that ratio would improve most sessions.

CagriSema phase 2 combination paper is one of those subjects where the general answer and the answer for a specific case diverge, and the thread will go in circles until someone says which one is being asked for.

Narrowing post #12, because the general version has more than one answer.

Bringing the previous paper on the same question makes the session much better and doubles the preparation. Worth doing every third session rather than every one.

I would not lead a decision with this, but I would not ignore it either.

Everything in post #14 holds. The case it does not cover is the one I have.

I disagree with the framing of CagriSema phase 2 combination paper above, and I think it is a substantive disagreement rather than a terminological one. Setting out why, so it can be checked.

The reasoning depends on an assumption that is doing a lot of work and is never stated. If the assumption holds, the conclusion follows. I do not think it holds generally.

Speaking only to CagriSema phase 2 combination paper as I have actually seen it, rather than as it is usually described: the effect is real, it is smaller than the thread suggests, and the variance between people is larger than the effect.

Where the group cannot agree, record the disagreement rather than resolving it by seniority. The recorded disagreement is more honest and more useful later.

That is the practical version. The rigorous version is longer and says the same thing.

The arithmetic in post #16 is right; the assumption feeding it is the part to check.

What would change my mind on CagriSema phase 2 combination paper is a second dataset collected by someone with no stake in the first. Until then I hold it loosely and I would rather say so than pretend to more.

The useful distinction on CagriSema phase 2 combination paper is between what was measured and what was inferred from it. Both end up in the same sentence and only one of them has error bars.

Reading order that works for these sessions: registry entry, methods, baseline table, primary result, then abstract last. Reading the abstract first anchors everything that follows.

That is one dataset and I would not build a rule on it.

Quoting the sentence rather than paraphrasing it changes the discussion. Paraphrase is where a claim quietly becomes stronger than the paper made it.

The answer changed when I changed how I was measuring, which was informative.

Worth separating two things that post #19 runs together.

TRIUMPH (ongoing): Retatrutide phase 3. No results yet. Nothing should be attributed to it because it has not finished. When it does, this discussion will return to it.

The baseline characteristics table is the single most useful page for the questions that get asked here, because it tells you who the result applies to.

Post #23 answers the question as asked. The question underneath it is different.

Counterpoint on CagriSema phase 2 combination paper, offered without confidence: the same observation is consistent with a much duller explanation, and nobody has ruled the dull one out.

An hour is enough for one paper read properly and not enough for two read partly. The temptation is always to add the second.

I am not the right person to answer the follow-up to this.

Where I part company with post #28, and it is a narrow parting.

Sessions run better when the paper is circulated with three specific questions attached. Open-ended reading produces an open-ended discussion.
