MHRA statements, read directly

Solved Pinned
Solved by s.okonkwo in post #2
Narrowing the opening post, because the general version has more than one answer. A note on how MHRA statements gets discussed rather than on MHRA statements itself: the confident posts get the replies and the careful ones get ignored, and the careful ones have been right more often.

MHRA statements, read directly Writing it up because I had to work it out twice and would rather nobody else did.

Asking about MHRA statements directly, because I have read four threads on it and each answered a slightly different question.

The version I want answered is the narrow one: given the method stated below, is the result within what anyone else has seen? I am not asking what it means yet.

Method, numbers and the two assumptions I am aware of making are below. If the assumptions are wrong that is more useful to me than agreement.

The arithmetic on MHRA statements is the easy part and it is where the errors are, which is an uncomfortable combination. Show your working and someone will catch it.

The most useful contribution is a link to the current official page with the date you accessed it. That survives; a paraphrase does not.

I would rather be precise about what I do not know than vague about what I do.

Where a member reports a decision, the useful detail is the stated reason rather than the outcome, because the reason is what generalises.

Flagging that the sources on this are thinner than the confidence in the thread suggests.

Where the MHRA statements discussion usually stalls is that nobody wants to say "I do not know" and everyone is willing to say "it varies". Those are the same sentence with different clothes on.

Worth separating two things that post #5 runs together.

Second-hand on MHRA statements, so weight it accordingly — someone whose method I trust told me this and I have not verified it myself.

Reading rather than contributing, but this is the most useful thread I have found on it.

Cross-border movement between the two jurisdictions is not the same as domestic movement within either, and the rules are published rather than inferred.

Noting that the question and the thing people usually mean by it are different.

Waiting times are the most variable and least generalisable thing reported here, and they are worth posting anyway with a region and a date.

Right — I had this wrong and I am glad to have read it before it mattered.

The arithmetic in post #11 is right; the assumption feeding it is the part to check.

The most useful thing anyone has posted about MHRA statements in this category was a table of what had been measured and by whom. That is what I would want again.

Anyone quoting an eligibility threshold should say which document it comes from. The thresholds have changed and the old ones are still circulating.

Two claims get bundled together under MHRA statements and they need separating. The descriptive one — this is what was observed — is usually well supported. The causal one — this is why — usually is not.

Almost every disagreement in threads like this one dissolves once you say which of the two you are making.

Adding the measurement that post #15 says would settle it.

I changed my mind about MHRA statements after someone here asked me for the source and I could not produce one. That is worth saying out loud because it is the ordinary way it happens.

Nothing in this subcategory is medical or legal advice, and prescribers reading here consistently say so themselves.

Coming back to post #16, because the follow-up matters more than the original answer.

The relevant national body publishes its decisions and its shortage notifications directly, and those are the primary sources rather than any secondary summary.

That is all I can say without guessing.

I keep a log for MHRA statements specifically because my memory of it turned out to be systematically wrong in one direction. Six weeks of notes cost nothing and settled it.

MHRA is the regulator. Licensed incretin analogues are prescription-only medicines. NHS and private prescribing routes exist with materially different access criteria and costs.

I would call that likely rather than established.

Before the thread moves on from MHRA statements — what is the sample size behind the claim? I am not being difficult; I have seen the same figure quoted from an n of four and from an n of four hundred.

That matches what I have seen, for whatever a single anecdote is worth.

Where I part company with post #21, and it is a narrow parting.

A referral pathway that worked eighteen months ago may not exist now. The archive keeps these posts and they are read as current.

I would rather post the uncertainty than round it away.

Adding the boring version of MHRA statements, because the interesting version keeps getting posted and the boring one is usually right.

Check the ordinary explanations, in order, and stop when one of them accounts for what you are seeing. Most of the time the second one does.

Where the compound is supplied with a device, the device and the compound have separate availability positions and the device is more often the constraint.

Post #25 put the caveat in the right place and I want to underline it.

The useful distinction on MHRA statements is between what was measured and what was inferred from it. Both end up in the same sentence and only one of them has error bars.

Building on post #29 rather than restating it.

Where guidance distinguishes between indications, quoting the indication is essential. The same compound has different positions under different indications.

A guess, clearly labelled as one.
