  • 12 Dec 2025 — dietitian_hollis: Added the limitations paragraph that review asked for.
  • 8 Dec 2025 — journalclub_wren: Restructured into sections so the outline is navigable.
  • 15 Jun 2026 — v.szabo: Plain-language pass on the opening paragraph.

Confirming the opening post from a second method, which matters more than confirming it from a second person.

What I would want before treating micro-titration as settled: the method, the sample, and whether anyone tried to find the opposite result. Two of the three are usually missing.

l.chevalier, post #9: Small correction to my own earlier position on micro-titration. I had the units the wrong way round, which changes the conclusion by an order of magnitude and therefore changes it entirely. Go to post

Post #17 put the caveat in the right place and I want to underline it.

I would keep micro-titration and the decision it usually gets used for separate in this thread. They are related and they are not the same question, and merging them is why the last one went badly.

e.silva, post #1: Micro-titration: a disputed topic, argued properly Writing it up because I had to work it out twice and would rather nobody else did. Two things I would like separated before anyone answers on micro-titration, because they get bundled and then argued about as one thing. The first is descriptive: what has actually been observed, by whom,… Go to post

The four-week escalation interval is a convention from the pivotal trials, not a pharmacological constant. The pharmacological argument is that with a week-long half-life, four weeks approaches steady state and you can assess the dose fairly. That is an argument for not going faster. It is not an argument against going slower.

This follows post #32 rather than contradicting it.

Two people in this thread mean different things by micro-titration and are disagreeing about the definition while believing they are disagreeing about the facts. Worth pausing to define it.

Concentration and dose get conflated constantly in this subcategory. Changing how much diluent you add changes the volume you draw and changes nothing about the dose.

Reading it again, the caveat matters more than the finding.

g.amankwah, post #31: Taking post #28 at face value and following it one step further. Genuine question rather than a rhetorical one: has anyone here actually observed micro-titration, as opposed to read about it? The thread is long and I cannot tell. Go to post

If you are new and reading this thread for the answer to micro-titration: the answer is conditional, the conditions are in the third reply, and the rest of the thread is worth skipping.

sharps_bin, post #36: Micro-titration: the concept of increments smaller than the labelled steps. It has no evidence base from trials but is described by some people. The downside is that very small increments are hard to measure accurately with a syringe. Go to post

A note on scope: what I am saying about micro-titration applies to the case in the first post and I would not extend it further without checking.

Micro-titration has a well-known answer and a correct answer, and the interesting work is establishing that they are the same. Nobody has done that here yet.

Writing down the date, the dose and the site each week takes fifteen seconds and is the single most useful record anyone here keeps. Memory reconstructs a titration history that never happened.

Happy to be corrected if someone holds better data than mine.
