Severe gastroparesis and why this class is a poor fit — one year on — setting out what I have, and where I think it stops being reliable.

Asking about Severe gastroparesis directly, because I have read four threads on it and each answered a slightly different question.

The version I want answered is the narrow one: given the method stated below, is the result within what anyone else has seen? I am not asking what it means yet.

Method, numbers and the two assumptions I am aware of making are below. If the assumptions are wrong that is more useful to me than agreement.

My experience of Severe gastroparesis contradicts the reply above. I am posting it as a data point rather than as a refutation, because one person's experience is exactly that.

Nothing here is medical advice, and a contraindication question is one where the cost of a wrong forum answer is high.

Confirming post #3 from a second method, which matters more than confirming it from a second person.

Severe gastrointestinal disease interacts with a delayed-emptying mechanism and appears in the labelling for that reason.

Somebody's condition being on a list does not settle the decision, and somebody's condition being absent from one does not either.

Adding it in case it saves somebody the afternoon it cost me.

The reason Severe gastroparesis keeps being re-asked is that the answer is conditional and people quote it without the condition. It is not that the answer is unknown.

Severe gastroparesis is well covered in the tag pages, and the older discussions are better than the recent ones because they were argued out properly. Worth twenty minutes before adding to this one.

Worth separating two things that post #7 runs together.

Asking a pharmacist to check a contraindication takes minutes and is more reliable than any thread, including this one.

Anyone posting a contraindication claim should include the source. Without one it circulates and hardens.

If anyone has run this properly I would rather read that than my own guess.

Genuine question rather than a rhetorical one: has anyone here actually observed Severe gastroparesis, as opposed to read about it? The thread is long and I cannot tell.

I read post #10 twice before replying, because I had assumed the opposite.

Allergy to the specific compound: true allergy is rare but possible. Any prior allergic reaction to the same compound or to structurally similar peptides warrants caution.

That is the version I use. It may not be the version that is correct.

Severe renal impairment (eGFR <15): these compounds are renally cleared and accumulate in severe kidney disease. Risk-benefit is unfavourable without dose adjustment.

I am not the right person to answer the follow-up to this.

Post #14 and I disagree about the size of the effect, not about the direction.

Severe hepatic impairment: less is known than for kidney disease. Extreme caution or contraindication depending on the specific degree of impairment.

The mechanism is plausible, which is not the same as established.

A methods point on Severe gastroparesis rather than a substantive one: if the comparison is not like for like, the difference you are measuring is the difference in method.

Post #18 put the caveat in the right place and I want to underline it.

A caution and a contraindication are different levels and the difference is not stylistic. Conflating them makes the labelling sound more restrictive than it is.

The confident version of this sentence would be wrong, so here is the hedged one.

Gastroparesis or severe delayed gastric emptying: these compounds slow gastric emptying. Pre-existing severe gastroparesis can be worsened. That is a relative contraindication depending on baseline severity.

Everything in post #20 holds. The case it does not cover is the one I have.

My understanding of Severe gastroparesis is a few years old and may have been superseded. If it has been, I would genuinely like to know rather than keep repeating it.

The claim about Severe gastroparesis upthread is stronger than its source supports. I have read the source. The source says "associated with" and the post says "causes".
