Pancreatitis history and the risk calculus

Solved
Solved by l.dziedzic in post #3
Picking up the opening post: that is the part I would want checked first. I keep a log for pancreatitis history specifically because my memory of it turned out to be systematically wrong in one direction. Six weeks of notes cost nothing and settled it.

Posting this under the heading it deserves: Pancreatitis history and the risk calculus Everything below is what sits behind that.

I would like to know what people here actually do about pancreatitis history, as distinct from what is usually recommended. Those have diverged in every other subject I have looked at closely.

Mine is below, with the reasoning, including the parts I am not confident about.

Right — I had this wrong and I am glad to have read it before it mattered.

Allergy to the specific compound: true allergy is rare but possible. Any prior allergic reaction to the same compound or to structurally similar peptides warrants caution.

Marking that as an opinion rather than a finding.

Medullary thyroid carcinoma (personal or family history) or multiple endocrine neoplasia type 2: absolute contraindication. The rodent preclinical signal is real enough to exclude this population.

I would be glad to be shown a cleaner way of putting this.

A history of pancreatitis appears in the cautions and the decision around it needs somebody who knows the history rather than a general rule.

A single observation, in a thread that deserves better than single observations.

This follows post #4 rather than contradicting it.

The confident answers on pancreatitis history and the well-sourced answers are not the same answers, which is the most useful thing I have learned reading this category.

Worth separating two things that post #6 runs together.

Somebody's condition being on a list does not settle the decision, and somebody's condition being absent from one does not either.

Narrowing post #8, because the general version has more than one answer.

A request rather than an answer: could whoever has the primary source for pancreatitis history post it? I have seen the claim three times this month and each version had lost a qualifier.

Source for the pancreatitis history figure, since it was asked for. It is in the discussion rather than the abstract, which is why the version circulating is stronger than the paper is.

Reading the surrounding paragraph is worth the two minutes. The authors are more careful than their summarisers.

Disordered eating history: appetite suppression can trigger relapse in people with history of anorexia, bulimia, or other eating disorders. This is a relative contraindication requiring specialist input.

Everything in post #9 holds. The case it does not cover is the one I have.

Pancreatitis history is well covered in the tag pages, and the older discussions are better than the recent ones because they were argued out properly. Worth twenty minutes before adding to this one.

Reading back through the pancreatitis history threads from last year, the same three questions come up every time and only one of them has ever been answered properly. That seems like a documentation gap rather than a knowledge gap.

Allergy to the specific compound: true allergy is rare but possible. Any prior allergic reaction to the same compound or to structurally similar peptides warrants caution.

That holds under the stated conditions and I have stated them.

Diabetic retinopathy complications: a signal for this was noted in SUSTAIN 6. Current evidence is mixed. The risk is not zero and some caution is appropriate in people with baseline retinopathy.

Answering the question post #13 raises rather than the one it answers.

History of pancreatitis: these compounds can rarely trigger pancreatitis. History of pancreatitis increases relative risk. That is a relative contraindication, not absolute, but requires monitoring.

That is what I would do. It may not be what is correct.

The arithmetic in post #17 is right; the assumption feeding it is the part to check.

Somebody's condition being on a list does not settle the decision, and somebody's condition being absent from one does not either.

I looked this up rather than remembered it, which is the right order.

On post #17 — agreed on the reasoning, with one qualification.

Allergy to the specific compound: true allergy is rare but possible. Any prior allergic reaction to the same compound or to structurally similar peptides warrants caution.

If it helps: the failure mode here is usually boring rather than dramatic.

Two people in this thread mean different things by pancreatitis history and are disagreeing about the definition while believing they are disagreeing about the facts. Worth pausing to define it.

Everything in post #21 holds. The case it does not cover is the one I have.

Allergy to the specific compound: true allergy is rare but possible. Any prior allergic reaction to the same compound or to structurally similar peptides warrants caution.

The variance between people here is larger than the effect being discussed.

Personal or family history of medullary thyroid carcinoma, or multiple endocrine neoplasia type 2, is the clearest contraindication in this class and it is specific rather than general.

The failure mode on pancreatitis history is boring rather than dramatic. It is almost always the step everyone assumes was done correctly because it is too simple to get wrong.

Pancreatitis history is a question about a distribution, not about a value, and treating it as a value is what produces the confident wrong answers.

Post #26 is the version of this I will quote in future. One addition.

Diabetic retinopathy complications: a signal for this was noted in SUSTAIN 6. Current evidence is mixed. The risk is not zero and some caution is appropriate in people with baseline retinopathy.

One case, stated as one case.

Where I part company with post #28, and it is a narrow parting.

Disordered eating history: appetite suppression can trigger relapse in people with history of anorexia, bulimia, or other eating disorders. This is a relative contraindication requiring specialist input.

Stating my assumptions rather than smuggling them in.
