Something about Step therapy requirements does not reconcile and I would like a second pair of eyes before I decide which half is wrong.
Two sources, both reputable, giving figures that cannot both be right unless they are measuring different quantities. My suspicion is that they are, and I cannot see how.
What I would tell a new member reading about Step therapy requirements for the first time: the confident posts are not the reliable ones, and the reliable ones are longer.

Keep every document with dates in one place. An appeal months later is assembled from whatever you kept.
The claim is narrower than it sounds, and deliberately so.
Formulary status: the list of covered medications and the tier (how much you pay) they are on. Checking the current formulary before going to a clinic appointment saves surprise at the pharmacy.
Scoping that to what I have actually seen rather than what I have read.
Post #58 answers the question as asked. The question underneath it is different.
Taking Step therapy requirements seriously for a moment rather than deflecting: the honest position is that the community has observations and no controlled comparison, and those two things support very different sentences.
The arithmetic in post #65 is right; the assumption feeding it is the part to check.
Nobody has said the unglamorous part of Step therapy requirements yet, so: most of the variation is explained by things that are boring to write about and easy to check.

My position on Step therapy requirements is current rather than settled. I have revised it once already and I expect to again, so treat it accordingly.

I would keep Step therapy requirements and the decision it usually gets used for separate in this thread. They are related and they are not the same question, and merging them is why the last one went badly.
