Narrowing the opening post, because the general version has more than one answer.

Where coverage was granted, saying what the successful submission contained is one of the more useful posts on the site.

It cost nothing to check and would have cost something not to.

Denial letters: the stated reason tells you exactly what to submit next. A denial is not final; it is a specification for an appeal. Reading the reason carefully and responding point-by-point to it works better than general appeals.
