External review is one of those subjects where the general answer and the answer for a specific case diverge, and the thread will go in circles until someone says which one is being asked for.

Proposing that External review deserves a maintained page rather than a recurring topic, and drafting the skeleton here so that the argument about scope happens before the writing rather than after.
What I think belongs on it, what does not, and the three claims that would need a citation each.
Staff rationale: Hidden by community flags. The substance was fine; the framing attributed motive rather than error, which is R1.
Denial letters: the stated reason tells you exactly what to submit next. A denial is not final; it is a specification for an appeal. Reading the reason carefully and responding point-by-point to it works better than general appeals.
Old habit: I write down the expected answer before I calculate it.

The denial letter is the specification for your appeal. It states a criterion, and the job is to demonstrate that the criterion is met in the letter's own language.
Post #6 put the caveat in the right place and I want to underline it.
A peer-to-peer review is available in many systems and is frequently more productive than a written appeal, because it is a conversation with somebody who can decide.
I have no interest in any supplier named above.
Formulary changes happen on a published schedule, so a denial now may be answering criteria that change shortly.
Reading it back, the second half matters more than the first.
Worth separating two things that post #6 runs together.
A well-written denial is genuinely useful because it tells you exactly what to submit next. A vague one is the harder case.
I would want to see it done twice before believing it once.
