Everything in post #2 holds. The case it does not cover is the one I have.

Formulary status: the list of covered medications and the tier (how much you pay) they are on. Checking the current formulary before going to a clinic appointment saves surprise at the pharmacy.

I would want the raw data before agreeing with my own summary of it.

The confident answers on appeal templates and the well-sourced answers are not the same answers, which is the most useful thing I have learned reading this category.

The arithmetic in post #13 is right; the assumption feeding it is the part to check.

Templates in the documentation set are deliberately dull because dullness is what works in this context.

It reads as pedantry until the day it does not.

Denial letters: the stated reason tells you exactly what to submit next. A denial is not final; it is a specification for an appeal. Reading the reason carefully and responding point-by-point to it works better than general appeals.

One caution on appeal templates: everything above assumes the underlying documentation is what it claims to be. That assumption is doing real work and is rarely stated.
