Hair shedding at month three: mechanism and time course — the long version Writing it up because I had to work it out twice and would rather nobody else did.

Reading back through what has been written here about Hair shedding, three questions come up every time and only one has ever been answered properly.

Listing all three, with what I think the state of the answer is for each, so the thread can start further along than the last one did.

I read the opening post twice before replying, because I had assumed the opposite.

Effects that improve with time and effects that improve with dose reduction are different findings. Working out which you have requires holding the dose steady long enough to see.

Everything in post #9 holds. The case it does not cover is the one I have.

A symptom that appeared at the same time as a dose change and disappeared when the change was reversed is the strongest evidence an individual can produce, and it is still an uncontrolled observation.

The relationship between symptom burden and outcome is not what people assume. Feeling worse is not evidence of a larger effect and feeling nothing is not evidence of an inactive preparation.

I looked this up rather than remembered it, which is the right order.

Post #17 is right about the mechanism and I think understates the practical bit.

The version of Hair shedding that I was taught turned out to be a teaching simplification. Useful, and not true in the way I had assumed it was.

g.tanaka, post #11: Dehydration is the mechanism behind more of the secondary complaints here than people expect. Reduced intake plus vomiting plus a warm week is a straightforward path to feeling dreadful. It is the sort of thing that seems obvious in retrospect and was not at the time. Go to post

The practical version of Hair shedding is three sentences long. The rigorous version is three pages and reaches the same conclusion with the conditions attached.

s.kravchenko, post #25: Gallbladder complications: rapid weight loss increases the risk of gallstone formation. The mechanism is not specific to this drug class. The risk is greatest in the first months when weight loss is most rapid. Go to post

Post #30 describes the usual case. This is about the unusual one.

Effects that improve with time and effects that improve with dose reduction are different findings. Working out which you have requires holding the dose steady long enough to see.

Reporting the observation and leaving the explanation open deliberately.

The published rates come from populations that were titrated on a defined schedule with clinical supervision. Rates from a forum are not comparable and are biased by who chooses to post.
