Adding the measurement that post #8 says would settle it.

Where a service has its own criteria layered on top of national guidance, those are the criteria that decide the outcome, and they are usually published.

Stating my assumptions rather than smuggling them in.

Trying to state the NHS criteria position in a way that someone who disagrees would recognise as fair, because I do not think the version in this thread passes that test.

Small correction to my own earlier position on NHS criteria. I had the units the wrong way round, which changes the conclusion by an order of magnitude and therefore changes it entirely.
