Where to report an adverse event officially, by region

Solved
Solved by i.ilunga in post #3
I had written a reply contradicting the opening post and deleted it. Here is what survived. Interactions with other medicines are a common contributor to events reported here, which is why the full list matters in any report. This has been discussed before and I could not find the thread, so, again.

Where to report an adverse event officially, by region Writing it up because I had to work it out twice and would rather nobody else did.

Reporting an event with the dates and the sequence, because attribution is impossible without them.

What happened, when, relative to the dose, and everything else that was in play at the time. I am not claiming causation and I would like to know what would establish it.

Nothing here is medical advice and I am describing my own case rather than asking anybody to diagnose it.

Useful. I had the fact and not the reason, which turns out to be the important half.
