[2026 update] Journal club: PIONEER 6 as a safety trial

Journal club: PIONEER 6 as a safety trial Writing it up because I had to work it out twice and would rather nobody else did.

Bringing a claim rather than a paper, because the claim is the thing circulating.

The statement gets made confidently and cites PIONEER 6 (N Engl J Med, 2019) when pressed. I have read the trial and I cannot find the sentence it is supposed to rest on.

Either I am reading the wrong section or the claim has drifted from the source. I would like to know which before I repeat the correction anywhere.

Filing a mild objection to the consensus on PIONEER 6. Mild because I might be wrong; an objection because nobody has addressed the case that does not fit.

Coming back to the opening post, because the follow-up matters more than the original answer.

Reading this PIONEER 6 thread as someone who came in with a fixed view: the third and seventh replies moved me and the confident ones did not.

The opening post is right about the mechanism and I think understates the practical bit.

The figures usually contain the finding and the text usually contains the interpretation. Separating them for the first twenty minutes is a discipline worth keeping.

Caveat: everything above assumes the paperwork is what it says it is.

On the opening post — agreed on the reasoning, with one qualification.

Where the group cannot agree, record the disagreement rather than resolving it by seniority. The recorded disagreement is more honest and more useful later.

I would rather say I do not know than round it up to an answer.

STEP 1 (N Engl J Med 2021): The pivotal obesity trial for semaglutide and the reference point for most subsequent comparison. Mean weight reduction was substantially larger than anything previously achieved pharmacologically.

The conclusion is tentative; the arithmetic underneath it is not.

Confirming post #5 from a second method, which matters more than confirming it from a second person.

The version of PIONEER 6 that I was taught turned out to be a teaching simplification. Useful, and not true in the way I had assumed it was.

Post #5 put the caveat in the right place and I want to underline it.

PIONEER 6 is a good example of a question where the honest answer is boring and the interesting answers are unsupported. I would go with boring.

The baseline characteristics table is the single most useful page for the questions that get asked here, because it tells you who the result applies to.

PIONEER 6 is well covered in the tag pages, and the older discussions are better than the recent ones because they were argued out properly. Worth twenty minutes before adding to this one.

Sessions on negative or null results are consistently the most instructive and the hardest to get anyone to attend.

It is worth checking rather than assuming, which costs nothing.

Marking my uncertainty on PIONEER 6 explicitly. I am confident about the direction, much less confident about the size, and not confident at all that it generalises past the case in the first post.

If someone has run PIONEER 6 properly I would rather read that than my own reconstruction of it. Posting mine only because the thread has gone quiet.

The most common failure mode is spending fifty minutes on the effect size and ten on the population. Reversing that ratio would improve most sessions.

Second-hand, so weight it accordingly.

A methods point on PIONEER 6 rather than a substantive one: if the comparison is not like for like, the difference you are measuring is the difference in method.

Two people in this thread mean different things by PIONEER 6 and are disagreeing about the definition while believing they are disagreeing about the facts. Worth pausing to define it.
