Lipase elevation without symptoms: the interpretation problem Writing it up because I had to work it out twice and would rather nobody else did.

Lipase elevation without symptoms — I have the observation and I do not trust my interpretation of it, so I am posting the observation and holding the interpretation back.

Numbers, method and the conditions under which they were collected are below. Interpret them however they warrant.

Narrowing the opening post, because the general version has more than one answer.

What makes a result interesting: a trend across draws, a magnitude well beyond the interval, a pattern that coheres with other values, or symptoms that fit.

A trend needs at least three points to be a trend. Two points are a line and a line through noise is still a line.

Noting that I have skin in this question and have tried to discount for it.

Answering the question the opening post raises rather than the one it answers.

TSH and free hormones: TSH is a reasonable screening test but does not tell you about actual free hormone levels. If TSH is abnormal, free thyroid hormones confirm whether there is a real thyroid problem.

I would call the community position on lipase elevation without symptoms likely rather than established, and I would be comfortable defending that hedge.

Building on post #5 rather than restating it.

Repeat before acting is standard practice for an isolated abnormal result on most analytes, and it is standard for a reason.

The honest answer on lipase elevation without symptoms is that it depends, and the useful part is the list of what it depends on. Four items, in rough order of how much they matter.

Most people get the first two right and then argue about the fourth.

Glycaemic markers integrate over different periods. A point glucose and a longer-term marker disagreeing is not a contradiction; it is two different questions.

I have separated what I observed from what I concluded, which does not always happen.

Time of day: some biomarkers vary across the day. Cortisol in the morning differs from cortisol in the evening. Comparing results from different times of day is comparing things that are not the same.

Adding the measurement that post #10 says would settle it.

Where the lipase elevation without symptoms reasoning breaks down for me is the step from the group result to the individual case. That step is almost never argued for.

A note on scope: what I am saying about lipase elevation without symptoms applies to the case in the first post and I would not extend it further without checking.

The arithmetic in post #14 is right; the assumption feeding it is the part to check.

Where a panel includes a calculated value, that value inherits the imprecision of everything it was calculated from and is usually reported without an interval.

That is the practical version. The rigorous version is longer and says the same thing.

Answering the question post #17 raises rather than the one it answers.

Liver enzyme changes during rapid weight loss are commonly reported and have several possible explanations, which is exactly why they need clinical interpretation rather than forum interpretation.

I would hold that lightly until someone with a larger sample weighs in.

Lipid panel interpretation: total, LDL, HDL, triglycerides all on one panel. Reading them together is more informative than reading one value in isolation. An elevated triglyceride with low HDL is different from triglyceride elevation alone.

Liver enzymes: elevation does not specify cause. ALT and AST can rise from many things. Bilirubin helps narrow down the cause. Multiple markers together are more informative than one alone.

Happy to be the one who is wrong here if it settles the question.

The failure mode on lipase elevation without symptoms is boring rather than dramatic. It is almost always the step everyone assumes was done correctly because it is too simple to get wrong.
