Before the thread moves on from decimal-point error — what is the sample size behind the claim? I am not being difficult; I have seen the same figure quoted from an n of four and from an n of four hundred.

Post #61 describes the usual case. This is about the unusual one.

Where I have landed on decimal-point error, having got it wrong once in public: the direction is clear, the magnitude is not, and anyone quoting a precise magnitude has borrowed it from somewhere that did not measure it.

Worth separating decimal-point error as a question about the compound from decimal-point error as a question about the documentation. They get answered by different people and only one of them is answerable here.

The question underneath decimal-point error is usually "how would I tell?" rather than "what is true?", and that one has a method attached to it.

Write down what you would expect to see under each hypothesis before you collect anything. If they predict the same observation, collecting it will not help.

I would put moderate confidence on the mainstream reading of decimal-point error and no more. That is not scepticism for its own sake; it is where the sourcing actually stops.

Answering the question post #70 raises rather than the one it answers.

A definition problem is doing most of the work in this decimal-point error discussion. Once the term is pinned down I suspect the disagreement mostly goes away and what is left is small.

Decimal-point error: I would want to see the raw numbers rather than the summary before agreeing. Summaries lose exactly the information that would settle this.

The decimal-point error: computing 5 mg / 2 mL as 0.25 mg/mL instead of 2.5 mg/mL is the most common arithmetic error in this subcategory. The habit that catches it: writing the units in every step of the calculation.

Anyone who has looked at this more carefully, please correct the record.

Confirming post #83 from a second method, which matters more than confirming it from a second person.

The number people quote for decimal-point error is a central estimate presented without its interval, and the interval is wide enough that the estimate is nearly uninformative on its own.

Write the reconstitution date and the concentration on the vial. Not on a note, on the vial. Every account here of a dosing error involving the wrong concentration involves a vial with nothing written on it.

What I would check first on decimal-point error is whether the thing being measured moved or whether the way of measuring it moved. Those look identical in a graph.
