The most common practical error is not the schedule at all — it is losing track of which step you are on after a break, and then resuming at the top rather than re-approaching it. Not the answer, but possibly the question that gets there.

Four-week escalation interval — I have the observation and I do not trust my interpretation of it, so I am posting the observation and holding the interpretation back.

Numbers, method and the conditions under which they were collected are below. Interpret them however they warrant.

I would be cautious about generalising from the four-week escalation interval example above. It is a good example. It is one example.

Post #2 put the caveat in the right place and I want to underline it.

A definition problem is doing most of the work in this four-week escalation interval discussion. Once the term is pinned down I suspect the disagreement mostly goes away and what is left is small.

Practical experience of four-week escalation interval, offered as one case with the conditions stated, not as a general finding. Conditions first, because they are what make it interpretable.

Taking post #4 at face value and following it one step further.

Going back down a step is not a failure and the trials allowed it. Several protocols permitted a return to the previous dose for tolerability and then a second attempt at the step.

My understanding of four-week escalation interval is a few years old and may have been superseded. If it has been, I would genuinely like to know rather than keep repeating it.

An observation about four-week escalation interval that I cannot explain and am posting anyway, on the principle that unexplained observations are more useful public than private.

Where I part company with post #9, and it is a narrow parting.

I have kept the units in throughout, for the obvious reason.

Four-week escalation interval was covered in the wiki last year and the page has a review date on it, which is a better starting point than my memory of a thread.

Nothing here is medical advice, and a titration question is one of the few where a prescriber can genuinely answer in a minute what a thread will take a week to circle.

Worth one more sentence than it usually gets.

The arithmetic in post #17 is right; the assumption feeding it is the part to check.

The reason four-week escalation interval is hard to answer is that the obvious measurement and the relevant quantity are not the same thing, and substituting one for the other is silent.

Splitting a weekly dose in two: the pharmacokinetic argument against is that you want the benefit of long half-life, which gives a slowly changing plasma level from a weekly dosing schedule. Splitting it flattens the curve further but loses the convenience of once-weekly dosing. The trade-off is convenience versus a slightly flatter concentration curve.

I would put this at better than even and not much better.

Worth stating the null on four-week escalation interval before we explain it: the observation may be nothing. That possibility deserves a sentence and usually does not get one.

Adding the measurement that post #26 says would settle it.

The four-week escalation interval is a convention from the pivotal trials, not a pharmacological constant. The pharmacological argument is that with a week-long half-life, four weeks approaches steady state and you can assess the dose fairly. That is an argument for not going faster. It is not an argument against going slower.

The general answer and the answer for your case may diverge here.

I read the earlier replies on four-week escalation interval twice before writing this, because I had assumed the opposite and wanted to be sure I was disagreeing with what was said rather than what I expected.
