On the subject in the title: Isotonicity and what happens when you get it wrong Working notes rather than a conclusion.

I was wrong about isotonicity in a thread last spring and I would like to correct it publicly rather than quietly.

The error was in the units, which changed the conclusion by an order of magnitude. Setting out the corrected version, and the way I now check for that class of mistake.

Worth stating the null on isotonicity before we explain it: the observation may be nothing. That possibility deserves a sentence and usually does not get one.

Building on post #2 rather than restating it.

Residual moisture in a lyophilised product is a stability determinant and is measured by Karl Fischer titration. Above a few per cent, shelf life shortens considerably.

I am aware this is the third time this month I have made this point.

The arithmetic on isotonicity is the easy part and it is where the errors are, which is an uncomfortable combination. Show your working and someone will catch it.

Where the isotonicity discussion usually stalls is that nobody wants to say "I do not know" and everyone is willing to say "it varies". Those are the same sentence with different clothes on.

Answering the question post #4 raises rather than the one it answers.

Counter-ion exchange from trifluoroacetate to acetate is a purification step with a cost, and material sold without it retains TFA. That affects mass balance and can affect biological assays.

Happy to be the one who is wrong here if it settles the question.

Adding the measurement that post #6 says would settle it.

Sterility and stability are separate properties requiring separate controls. A preservative addresses one of them and nothing about the other.

Adding this to the thread rather than to the wiki, because I am not confident enough for the wiki.

Post #8 describes the usual case. This is about the unusual one.

Counterpoint on isotonicity, offered without confidence: the same observation is consistent with a much duller explanation, and nobody has ruled the dull one out.

Isotonicity matters for comfort at injection and does not matter for stability. Those two get conflated when people compare diluents.

Taking post #11 at face value and following it one step further.

The bit of isotonicity that nobody enjoys is that the answer changes depending on what you are trying to decide with it. Say what the decision is and the thread will converge.

Light sensitivity varies enormously between peptides depending on the residues present. Tryptophan-containing sequences are the ones where it matters most.

Everything in post #15 holds. The case it does not cover is the one I have.

Something worth flagging about isotonicity: the strongest-sounding claims in this thread are the ones with no source attached, which is the usual pattern and not a coincidence.

I have three months of notes on isotonicity and the honest summary is that the trend is real and the week-to-week numbers are noise. I nearly drew the opposite conclusion from the first fortnight.

Aggregation is concentration-dependent, which is why reconstituting to a high concentration is not always the convenient choice it appears to be.

I would put the burden of proof on the interesting explanation, not the dull one.

The version of isotonicity that circulates here is a simplification of a simplification. It is not wrong, but it has lost the conditions under which it holds, and those conditions are where the interesting cases live.

Overfill in a vial is deliberate and accounts for the volume unrecoverable at the end. It is not generosity and it is not a bonus dose.

The conclusion is tentative; the arithmetic underneath it is not.

A methods point on isotonicity rather than a substantive one: if the comparison is not like for like, the difference you are measuring is the difference in method.

Emulsification and suspensions: some formulations are emulsions or suspensions rather than solutions. Semaglutide formulations are solutions; some other compounds are suspensions. The physical form affects stability and administration.

The arithmetic in post #24 is right; the assumption feeding it is the part to check.

If someone has run isotonicity properly I would rather read that than my own reconstruction of it. Posting mine only because the thread has gone quiet.

The difference between a formulated pharmaceutical product and a lyophilised research powder is most of the engineering, and almost none of it is visible in a purity figure.

That distinction has done more work for me than anything else in this category.

Marking my uncertainty on isotonicity explicitly. I am confident about the direction, much less confident about the size, and not confident at all that it generalises past the case in the first post.

Isotonicity is well covered in the tag pages, and the older discussions are better than the recent ones because they were argued out properly. Worth twenty minutes before adding to this one.

Excipients and their purposes: mannitol, sucrose, or other sugars in lyophilised formulations protect the peptide during drying and storage. They are not impurities; they are functional components.
