A follow-up question about foaming during reconstitution that I did not know to ask the first time.

The earlier thread answered what I asked. What I should have asked is below, and I think it is the one that matters.

Reframing foaming during reconstitution slightly, because I think the disagreement is about the question rather than the answer. If the question is "does it happen", yes. If it is "how often", nobody here knows.

The bit of foaming during reconstitution that nobody enjoys is that the answer changes depending on what you are trying to decide with it. Say what the decision is and the thread will converge.

Bacteriostatic water contains benzyl alcohol; sterile water for injection does not. The difference matters when a container will be entered repeatedly over days and matters much less for a single-use preparation.

Reading it back, the second half matters more than the first.

Adding what did not work for me on foaming during reconstitution, since the failures never get written up and they are half the useful information.

Write the reconstitution date and the concentration on the vial. Not on a note, on the vial. Every account here of a dosing error involving the wrong concentration involves a vial with nothing written on it.

I would hold that lightly until someone with a larger sample weighs in.

Add the diluent down the side of the vial rather than directly onto the cake. It is slower and it avoids the foaming that makes people think something has gone wrong.

Two sources, same conclusion, and I could not rule out that one copied the other.

Worth separating two things that post #7 runs together.

The reason foaming during reconstitution is hard to answer is that the obvious measurement and the relevant quantity are not the same thing, and substituting one for the other is silent.

I had written a reply contradicting post #10 and deleted it. Here is what survived.

Do not shake. Swirl, or leave it. Vigorous agitation introduces air and shear, and neither helps a peptide go into solution any faster than patience does.

The step people skip is the one I have spelled out.

Confirming post #10 from a second method, which matters more than confirming it from a second person.

What I can speak to on foaming during reconstitution is narrow, so I will keep it narrow rather than generalising from it. Beyond that boundary I do not know.

Preservative effectiveness is tested against a defined microbial challenge under defined conditions. It is not a licence to treat an entered vial as sterile indefinitely, and no supplier claims otherwise.

Post #13 is right about the mechanism and I think understates the practical bit.

On foaming during reconstitution I would separate what is worth knowing from what is worth acting on. The first list is long and the second is short, and conflating them is how threads get heated.

The honest answer on foaming during reconstitution is that it depends, and the useful part is the list of what it depends on. Four items, in rough order of how much they matter.

Most people get the first two right and then argue about the fourth.

How much of the diluent volume the powder itself displaces: for a small peptide vial, the powder volume is negligible. For a larger vial or a kit with multiple compounds, the displacement can be a few tenths of a millilitre. If precision matters to you, account for it by targeting a final weight rather than a final volume.

Marking my uncertainty on foaming during reconstitution explicitly. I am confident about the direction, much less confident about the size, and not confident at all that it generalises past the case in the first post.

The best check on any reconstitution calculation is to do it twice by two different routes — mass per volume, then volume per dose — and see whether they agree. They should, and when they do not it is nearly always the concentration step.

Two people can read the same figure differently here and both be reasonable.

Two people in this thread mean different things by foaming during reconstitution and are disagreeing about the definition while believing they are disagreeing about the facts. Worth pausing to define it.
