A methods question rather than a substantive one, about renal impairment.
Everyone quotes the same figure and I cannot find anyone who says how it was arrived at. That is not an accusation; it usually means the derivation is somewhere obvious and I have missed it.
Everything in the opening post holds. The case it does not cover is the one I have.
Where a labelling document addresses a population explicitly, that text is the single most useful thing to quote in a thread.
That is one dataset and I would not build a rule on it.
Hepatic impairment: less is known about dosing in significant liver disease than in kidney disease. Extreme caution applies because the liver metabolises a large fraction of many medications.
The arithmetic in post #2 is right; the assumption feeding it is the part to check.
Hypoglycemia risk: in people already on insulin or sulfonylureas, adding a GLP-1 agonist requires insulin dose reduction and close monitoring for hypoglycemia. This is manageable with attention.
I would be glad to be shown a cleaner way of putting this.
