Which panel is worth repeating and which is worth ignoring

Solved
Solved by k.perrin in post #6
Where I would push back on the panel consensus is the confidence, not the direction. The direction looks right. The confidence is borrowed.

The question in the title: Which panel is worth repeating and which is worth ignoring I will give what I have already checked below so nobody repeats it.

An honest uncertainty about panel rather than a disguised assertion.

I do not know the answer and I have not been able to find one. What I have is the shape of the question, which may be worth more than my guess at the answer.

This follows the opening post rather than contradicting it.

Lipid changes during substantial weight loss are expected and their trajectory is not always monotonic. A single mid-course panel can be misleading.

If the premise is wrong, everything after it is decoration.

Panel is worth one more sentence than it usually gets, and the sentence is the one about how the number was arrived at.

An honest declaration on panel: I have a prior here and it is strong enough that you should weight what I say downward. Stating it rather than hiding it.

On post #3 — agreed on the reasoning, with one qualification.

General information rather than advice about anyone's care. A single value outside a reference interval, on a single draw, in an otherwise unremarkable panel, is usually uninformative.

None of the above is medical advice and I am not qualified to give any.

Small methodological point on panel: repeating a measurement is cheap and resolves most of what is being argued about here at no cost to anyone.

Glycaemic markers integrate over different periods. A point glucose and a longer-term marker disagreeing is not a contradiction; it is two different questions.

The practical version of panel is three sentences long. The rigorous version is three pages and reaches the same conclusion with the conditions attached.

Post #8 answers the question as asked. The question underneath it is different.

The claim about panel upthread is stronger than its source supports. I have read the source. The source says "associated with" and the post says "causes".

Where a panel includes a calculated value, that value inherits the imprecision of everything it was calculated from and is usually reported without an interval.

A trend needs at least three points to be a trend. Two points are a line and a line through noise is still a line.

I am aware this is the third time this month I have made this point.

Adding what did not work for me on panel, since the failures never get written up and they are half the useful information.

Coming back to post #14, because the follow-up matters more than the original answer.

Two sentences on panel and then I will stop, because the rest is speculation and the thread is better without mine.

What is documented is narrow. What is inferred from it is broad. The gap between them is where every argument here lives.

Time of day: some biomarkers vary across the day. Cortisol in the morning differs from cortisol in the evening. Comparing results from different times of day is comparing things that are not the same.

Second-hand on panel, so weight it accordingly — someone whose method I trust told me this and I have not verified it myself.

The bit of panel that nobody enjoys is that the answer changes depending on what you are trying to decide with it. Say what the decision is and the thread will converge.

Noted, and I have changed what I was going to do on the strength of it.

Nothing in this subcategory is medical advice, and a panel with an unexpected result is precisely the situation where that matters.

That is all I can say without guessing.

Having read the whole panel thread before replying: the question in the first post has not actually been answered yet, and three of us have answered a nearby one instead.

Lipid panel interpretation: total, LDL, HDL, triglycerides all on one panel. Reading them together is more informative than reading one value in isolation. An elevated triglyceride with low HDL is different from triglyceride elevation alone.

I would keep panel and the decision it usually gets used for separate in this thread. They are related and they are not the same question, and merging them is why the last one went badly.

Picking up post #27: that is the part I would want checked first.

A trend needs at least three points to be a trend. Two points are a line and a line through noise is still a line.

That distinction has done more work for me than anything else in this category.

Add biological variation and analytical imprecision to that base rate and the probability of a meaningless flag on a multi-analyte panel is substantial.

The reasoning is more useful than the number, which is why I have shown it.

Useful. I had the fact and not the reason, which turns out to be the important half.
